Youth Housing Coordinator - YHHS

Sabathani Community Center (SCC) is one of Minnesota’s oldest African American-founded nonprofits & serves a multi-cultural community of more than 50,000 community members in South Minneapolis. SCC was founded in 1966 by a handful of concerned community members seeking to address social and economic disparities in underserved communities in the areas of youth programming, housing, health and education. About the role

  • Youth Housing Coordinators provide Permanent Supportive Housing using scattered site locations within Hennepin County to Eligible Persons ages 16-24, who are at risk of or experiencing homelessness. The program will be paired with holistic, collaborative support services, including case management tailored to the needs of individuals that support long-term stability, self -sufficiency, and the ability to maintain independent living.

What you'll do

  • Direct Support and Documentation* 85%

· Receive referrals via Hennepin County CES referral process.

· Screen and enroll all eligible referrals for services.

· Assist eligible households with creating goals and support them with maintaining all program and reporting requirements.

· Connect interested applicants to Sabathani’s programs to provide a holistic service.

· Assist households with accessing Sabathani, collaborative, and community partners with necessary resources to maintain housing, including vocational, educational, and housing resources.

· Provide on-going case management services.

· Ensure all files are maintained and in compliance with the funder.


Internal Support* 10%

· Works closely with Sabathani’s staff to promote the housing assistance programs among the different departments.

· Works with Sabathani staff to capitalize on integrated programming/internal resources.

· Represents and promotes Sabathani’s mission and programs in a variety of public settings (outreach sites, events, organizations, referring agencies, etc.)

· Educate Sabathani Staff about the application requirements and application process.

Qualifications

  • Education and Experience

· High school diploma or equivalent required, Bachelor’s Degree

in human services, social work or related field, preferred.

· Minimum of 2 years of work experience in housing or social services.

· Experience working in a nonprofit setting preferred.


Knowledge, Skills and Abilities:

· Willingness to learn a variety of tasks; possesses excellent communication skills, a thorough understanding of nonprofit practices, a serious work ethic, and a desire to work in a team-orientated environment.

· Possesses the ability to work independently, establish priorities, and meet deadlines.

· Understands the importance of customer service and being professional.

· Knowledge and experience with diverse groups of people of lower socioeconomic conditions.

· Demonstrate skills in conflict management.

· Competency in MS Office: Word, Access, Publisher, Outlook, Excel, and PowerPoint.

· Strong organizational and communication skills with the ability to manage multiple priorities.

· Ability to maintain a good working relationship with all co-workers and the public. Must use good judgment in recognizing the scope of authority.

· Ability to travel to sites within 15-mile radius of the office.


Licenses and/or Certifications:

· Valid MN Drivers’ License, access to reliable transportation and the ability to meet Sabathani’s driving record requirements.
