Job Overview – Clinical Pharmacist (PharmD)

The Clinical Pharmacist (PharmD) functions as a trusted health advisor and behavior-change coach; someone patients look forward to hearing from. You will use clinical expertise as the foundation, but your primary skill is building enough trust with patients that they want to stay engaged in their care, disclose barriers and follow through on recommendations.

You will conduct MTM services, in particular Comprehensive Medication Reviews (CMRs), for eligible patients. Your real primary responsibility is identifying and addressing the full range of issues — clinical, behavioral, social, financial, logistical — that keep a patient from taking their medication and staying healthy. You will also establish protocols to help the team navigate and respond to different situations that may arise with patients, and liaise with non-clinical and clinical staff members to ensure continuity of care, and resolution of medication challenges during our program. The ideal candidate will demonstrate supportive, motivating, and expert patient and provider engagement, and demonstrate thought leadership in areas for further optimization of pharmacy services.


Responsibilities Include:

  • Build durable, trust-based relationships with patients through telephonic and video engagement; acting as a consistent, human touchpoint in their care, not a one-time transaction
  • Deliver MTM services (CMRs, targeted reviews, medication reconciliation, prior auth review) as a starting point for a broader conversation about the patient’s life, barriers, and goals
  • Proactively screen for and address social determinants of health (transportation, cost, food insecurity, health literacy, caregiver burden, mental health) that affect adherence, and connect patients to appropriate resources or teammates
  • Use motivational interviewing and behavior-change techniques, not just clinical scripts to understand why a patient is nonadherent and tailor the approach accordingly
  • Actively sell patients on the value of the vDOT program itself: articulate why daily engagement matters, address skepticism or fatigue, and re-engage patients who are drifting away
  • Assess the use of prescribed medication therapy for unnecessary medications, additional medication needs, inappropriate dosing, adverse drug reactions, ineffective medications, and adherence issues
  • Identify non-medication health issues (missed follow-ups, gaps in care, undiagnosed symptoms, mental health concerns) surfaced during conversations and route them to the right team member or provider
  • Document clinical recommendations and patient narrative context (not just clinical facts) so the care team can maintain continuity and personalize future outreach
  • Send clinical recommendations to provider offices and follow up on the responses to ensure recommendations are enacted
  • Coordinate with Scene Team, including nurses, health coaches, management, product managers, and client-facing staff as needed
  • Serve as a model of engagement style for other clinical staff; contribute to coaching frameworks/scripts that improve conversion and retention across the team
  • Introduce and explain our video-based direct observation therapy program to members, and assist them in starting their video submissions.
  • Record asynchronous videos and engage with members via video and chat to summarize key next steps from the medication review, respond to clinical questions, and encourage continued participation in Scene’s program

Required Qualifications

  • Active Pharmacist License with at least 3 years of pharmacy practice
  • Medically fluent in English and Spanish (native Spanish fluency preferred)
  • Demonstrated experience in a role requiring sustained patient relationship-building; health coaching, care management, telehealth, chronic disease management, or similar 
  • Comfort and skill with motivational interviewing, behavioral health screening, or coaching frameworks (training can be provided, but some prior exposure or aptitude expected)
  • Experience in digital health, health startups, health plans, or hospital quality improvement
  • Strong telephonic/video presence: warm, clear, unhurried communication style suited to the elderly population
  • Demonstrated ability to influence patient behavior — not just inform it
  • Ability to work autonomously in a virtual/decentralized team with minimal script-following


Preferred Qualifications

  • Native fluency in Spanish
  • Experience in health coaching, motivational interviewing certification, or behavioral health background
  • Sales, account management, or retention/conversion experience
  • Experience with value-based care, population health, or SDOH-focused programs
  • Track record of being described by patients or colleagues as “the person who really listens”


What Success Looks Like in This Role

  • Patients re-engage with and stick with the vDOT program because of the relationship, not just the reminder
  • Non-adherence root causes are identified and addressed beyond “did they fill the prescription”
  • Clinical recommendations to providers reflect a full picture of the patient, not just the med list
  • Other issues affecting health outcomes (missed screenings, care gaps, mental health) get surfaced and routed, even when outside strict MTM scope
