Lead Social Care Manager

About the Role – Social Care Manager (Lead)

The Social Care Manager (Lead) provides operational leadership and day-to-day supervision of Health-Related Social Needs (HRSN) services at StrongSteps Behavioral Health. This position oversees Care Managers, Intake staff, Housing staff, and support personnel to ensure members receive high-quality, timely, and compliant services related to care management, housing support, transportation coordination, food access, and community referrals.

The Social Care Manager (Lead) is responsible for monitoring staff productivity, documentation quality, service delivery standards, and member engagement while supporting team development and operational efficiency. This role works closely with agency leadership to maintain compliance, improve workflows, and support program growth across multiple service areas.

Responsibilities

  • Supervise and support Care Managers and support staff
  • Monitor staff productivity, caseload activity, and member engagement
  • Conduct regular staff meetings, supervision sessions, and performance reviews
  • Review documentation for quality, timeliness, and compliance standards
  • Assist with workflow coordination and operational problem-solving
  • Support intake coordination, housing services, transportation services, and community resource navigation
  • Address member concerns, escalations, and service barriers
  • Ensure compliance with Medicaid, HRSN, SCN, and organizational requirements
  • Collaborate with leadership regarding staffing, service delivery, and operational improvement
  • Assist with audit readiness, reporting, and quality assurance initiatives
  • Support training and onboarding of new staff members

Qualifications

  • Strong leadership, communication, and organizational skills
  • Experience in behavioral health, social services, healthcare, care management, or program supervision preferred
  • Ability to manage staff, workflows, and multiple operational priorities
  • Strong documentation review and problem-solving abilities
  • Comfortable working in fast-paced and evolving service environments
  • Basic computer and electronic documentation experience required
  • Knowledge of Medicaid, HRSN, SCN, or community-based services preferred

Employment Details

  • Full-Time Position
  • Salaried or Hourly Exempt Based on Experience
  • Starting Pay: $60,000–$65,000/year
