Job Title: Clinical Case Manager

Department: Clinical Services

Reports To: Chief Executive Officer


Position Summary

The Program Manager is responsible for overseeing the member’s overall care while in services with Benevolent Family Services. This role ensures that high-quality care is provided in compliance with regulatory requirements and best practice standards. The Program Manager will supervise staff, manage program activities, and ensure the efficient delivery of services to clients.

Key Responsibilities

  1. Staff Oversight
    • Conduct bi-monthly meetings with staff, documenting all sessions in the electronic health record (EHR) system.
    • Meetings will discuss their abilities to manage client care, sessions, and documentation
    • Assist in organizing and lead monthly staff meetings, which can count as one of the bi-monthly supervision sessions if attended in full.
  2. Client Care and Coordination
    • Ensure treatment plans are developed within 24 hours of assessment and updated every as required for services.
    • Coordinate with assessment team on individuals' needs and level of care
    • Conduct reviews of client goals and treatment prior to reauthorization or step-down during treatment team meetings.
    • Oversee medication management oversight with nurse practitioners and therapy provided by licensed professionals (LPC, LCSW, LMFT, LMHP-E).
    • Coordinate with nurses (LPN or RN) for nursing screenings and assessments
    • Complete care coordination and case management for members, ensuring they're prepared for discharge/step down. 
  3. Discharge Planning
    • Manage the discharge process, ensuring clients transition successfully to appropriate levels of care.
    • Ensure discharge documentation is completed and signed by all parties in the EHR.
    • Ensure all members have copies of their treatment forms and access to their portal
  4. Compliance and Documentation
    • Ensure all progress notes and documentation are submitted by the end of the business day of the day the service is completed.
    • Conduct 5 chart audits weekly to ensure compliance with state and federal regulations.
    • Maintain accurate records of supervision, attendance, and client progress in the EHR.
  5. Meet with Individuals 
    • Conduct treatment team meetings and one-on-one sessions with members of center-based services to gather information about their care needs and service goals.

Qualifications

  • Education: Master’s degree in a relevant field (e.g., Psychology, Social Work, Counseling) preferred.
  • Experience: Minimum of 3-5 years of experience in a clinical setting, with at least 2 years in a supervisory or management role.
  • Licensure/Certification: Must hold appropriate licensure or certification as required (e.g., CSAC, LMHP type preferred).
  • Skills: Strong leadership, communication, and organizational skills. Proficient in electronic health record systems and Microsoft Office Suite.
  • Attributes: Ability to work independently and as part of a team, strong problem-solving skills, and a commitment to providing high-quality care.

Working Conditions

  • Full-time position with typical hours from 8:00 AM to 4:30 PM, Monday through Friday.
  • This position is a hybrid and will require in-office days for one-on-ones and staff meetings.
