
The Care Navigator supports our clinical pharmacists in delivering Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services. Working across multiple North Carolina clinics, the Care Navigator helps manage a shared patient panel of 500 or more, serving as a consistent point of contact for patients between visits and a reliable second set of hands for the clinical team.
This is a part-time, on-site position based out of our Raleigh, NC location. Candidates must currently hold a nationally recognized Medical Assistant (MA) or Licensed Practical Nurse (LPN) certification and be able to provide a copy upon hire.
• Enroll eligible patients into RPM or CCM programs.
• Prepare patient charts ahead of upcoming CCM encounters.
• Make monthly check-in calls to patients enrolled in CCM.
• Make daily compliance calls to patients enrolled in RPM.
• Communicate urgent health concerns, medication refill requests, and appointment requests to clinical pharmacists promptly and accurately.
• Protect patient confidentiality at all times, and treat every patient with courtesy and respect.
• Current MA or LPN certification (required; documentation must be provided).
• High school diploma or GED (minimum requirement).
• Professional, confident telephone communication skills.
• Working knowledge of Excel, Allscripts or comparable EHR platforms, and general data entry.
• Strong time management and the ability to juggle competing priorities.
• Clear verbal and written communication paired with genuine customer service.
• A collaborative, team-first mindset paired with self-motivation and compassion.
• Working knowledge of medical terminology.
• Sound judgment and a calm, effective response in urgent or emergency situations.
• Ability to build effective working relationships with patients, caregivers, teammates, and providers.
• Prior experience with CCM or RPM programs.
• Prior pharmacy experience.
