Senior Backend Engineer (.NET, Full-Stack)

Company Description:

FluidAI is specialized in the development and deployment of data-driven surgical solutions, with an initial focus on general and gastrointestinal surgery. Our technologies integrate medical devices, software, and artificial intelligence into platforms that address real clinical and operational challenges across the continuum of surgical care.

We combine continuous physiological monitoring from our proprietary sensors with comprehensive electronic health record data, creating intelligent systems that enable earlier identification of clinical deterioration and complications. Beyond patient monitoring, our platform extends to patient education and engagement tools, as well as automated documentation and coding modules — supporting care teams and driving operational efficiency.

Our team brings together expertise across clinical practice, research, engineering, data science, manufacturing, and commercialization. We build solutions designed for real-world impact, supported by full in-house capabilities, a strong intellectual property portfolio, and partnerships with leading institutions around the world.

We are an equal opportunity employer and have a diverse team representation across language, ethnicity, gender and nationality.

Why Join FluidAI Medical?

  • Mission to save lives and improve patient care with technology that’s already deployed on patients in real-world settings.
  • Work with a high-performing and cross-functional team that is empowered to grow, learn, and contribute across all levels of the organization.
  • Gain exposure across the entire technical stack, from novel sensing technologies and proprietary hardware to mobile/web software and ML algorithms.
  • Personal career-focused development and training with access to workshops, coursework, and experienced mentors/consultants.
  • Regular offsite activities – past events include rafting, cottaging, axe-throwing, sports leagues, and the usual BBQ.

Job Summary:

FluidAI Medical is at the forefront of integrating data, technology, and healthcare to improve patient outcomes. Our platform connects proprietary sensing hardware, mobile and web applications, and ML algorithms to deliver real-time clinical insights at the bedside. We are expanding our team to include a Senior Backend Engineer who can own the design and evolution of the backend systems that power this platform, from device data ingestion and clinical APIs to the integrations, infrastructure, and reliability that a medical-grade product requires.

We work in a full-stack environment, where engineers own features end-to-end. Alongside backend ownership, this role works directly in our React, Next.js, and React Native codebases. That includes building and connecting the APIs, data flows, and real-time updates that bring our web and mobile applications to life.

This role is ideal for a senior, hands-on engineer with deep expertise in C# and .NET, a strong foundation in distributed systems and backend architecture, and practical experience shipping frontend and mobile features. Experience building software in healthcare or other regulated environments is highly preferred.

Note: This position is remote but requires travel across the Middle East to collaborate with the FluidAI team and clients.

Job Responsibilities:

Backend & Systems

  • Design, build, and own scalable, secure, and highly available backend services and APIs using C#, .NET, and ASP.NET Core.
  • Architect systems for ingesting, processing, and storing high-frequency sensor and device data reliably, including real-time and near-real-time pipelines.
  • Lead technical design for new services and major features, making sound decisions around service boundaries, data models, messaging, and scalability.
  • Design and optimize relational and time-series data stores (e.g., SQL Server, PostgreSQL), including schema design, query performance, and data integrity.
  • Design, build, and maintain solutions on NoSQL databases (e.g., MongoDB, Azure Cosmos DB, Redis), including document modeling, indexing strategies, and choosing the right data store for each workload.
  • Build and maintain event-driven and asynchronous workflows using message brokers and queues (e.g., Azure Service Bus, RabbitMQ, Kafka).
  • Develop integrations with hospital and third-party systems, including EHR/EMR platforms, using healthcare interoperability standards such as HL7 and FHIR.
  • Own system reliability and operability: logging, monitoring, alerting, tracing, and incident response for production services deployed in clinical settings.
  • Build and maintain cloud infrastructure, containerized deployments, and CI/CD pipelines that support safe, repeatable releases.

Full-Stack & Integration

  • Work across our full-stack codebases, delivering features end-to-end from database and API through to web and mobile user interfaces.
  • Build and maintain frontend features in React and Next.js, and mobile features in React Native, following established patterns and contributing to shared components where needed.
  • Connect backend services to frontend and mobile clients: design API contracts, implement authentication and authorization flows, manage data fetching and state, and handle error and offline scenarios.
  • Implement real-time data delivery to web and mobile clients (e.g., SignalR, WebSockets, push notifications) so that clinicians see device and patient data as it happens.
  • Partner with frontend, mobile, and design teammates to shape APIs around how the product is used, and keep backend and client code consistent and well-tested.
  • Debug and resolve issues across the full stack, tracing problems from the UI through APIs, services, and data stores.

Engineering Practice & Collaboration

  • Apply security, privacy, and compliance best practices throughout the software lifecycle, especially when handling sensitive patient and clinical data.
  • Write and maintain technical documentation, including architecture decisions, API specifications, and design artifacts that support regulatory and quality requirements.
  • Mentor engineers, drive code review standards, and raise the engineering bar across the team.
  • Participate in sprint planning and Agile ceremonies, contributing to roadmap planning and prioritization from a systems and architecture perspective.
  • Collaborate closely with hardware, firmware, ML, data, and clinical teams to deliver features end-to-end across the full technology stack.

Qualifications

  • 7+ years of professional software engineering experience, with significant time spent building production backend systems.
  • Expert-level proficiency in C# and .NET (.NET 6+ / ASP.NET Core), including Web API development, dependency injection, async programming, and Entity Framework Core or similar ORMs.
  • Strong background in distributed systems and backend architecture, including microservices or modular monoliths, REST and/or gRPC APIs, caching, and event-driven design.
  • Advanced SQL skills and strong data modeling experience with relational databases such as SQL Server or PostgreSQL, plus hands-on experience with NoSQL databases such as MongoDB.
  • Hands-on experience with a major cloud platform (Azure preferred; AWS or GCP considered), including managed services, networking, and identity.
  • Experience with Docker, Kubernetes or similar container orchestration, infrastructure as code, and CI/CD tooling.
  • Solid understanding of application security, including authentication and authorization (OAuth 2.0, OpenID Connect), encryption, and secure handling of sensitive data.
  • Experience with observability and production operations: structured logging, metrics, distributed tracing, and on-call/incident response.
  • Proven experience working in a full-stack environment, with hands-on production work in TypeScript, React, and Next.js.
  • Experience building or contributing to mobile applications with React Native.
  • Strong understanding of how frontend and backend fit together, including API design for client consumption, state management, auth flows, and real-time communication.
  • Experience working with IoT, sensor, time-series, or high-volume streaming data is a strong plus.
  • Prior experience in healthcare, medical devices, or digital health is highly preferred, including familiarity with HL7/FHIR, and with privacy and regulatory frameworks such as HIPAA, PHIPA, or Saudi PDPL.
  • Familiarity with medical device software standards (e.g., IEC 62304, ISO 13485) or experience working in other regulated environments is a plus.
  • Fluent in English; Arabic is a strong plus. Ability to communicate technical decisions clearly to diverse stakeholders.
  • Comfortable working in cross-functional, fast-paced, and collaborative environments.
  • Strong problem-solving mindset with a bias toward impact, ownership, and continuous improvement.
