Our accredited Diabetes Self-Management Education and Support (DSMES) program—aligned with ADA and ADCES standards—delivers personalized care through live telehealth sessions led by experienced Certified Diabetes Care and Education Specialists (CDCESs) and Nurse Practitioners.

Remote Telehealth

Location: Remote (U.S.-based)

Employment Type: Full Time Employee

Work Model: 100% Telecommute

Reports To: Vice President of Clinical Operations

Compensation: 

$86,000 to $90,000 annually
(Compensation is commensurate with experience)

Position Overview

  • The Team Lead – Diabetes Educators and Dietitians is a leadership role responsible for overseeing and supporting a remote team of Diabetes Educators and Dietitians while continuing to provide direct patient care. This individual combines strong clinical expertise with leadership and operational management skills to drive high quality patient outcomes, team development, and operational excellence.
  • The Team Lead partners closely with leadership to support strategic initiatives, optimize workflows, improve team performance, and enhance the patient experience across My Diabetes Tutor’s virtual care programs. This role is ideal for a clinician who is passionate about mentorship, collaborative leadership, and expanding access to high quality diabetes care through telehealth.

Remote Environment Requirements

  • Reliable computer with webcam, microphone, and high speed internet connection
  • Appropriate hardware including computer or laptop, webcam, and headset
  • Quiet, dedicated workspace that ensures confidentiality during telehealth visits and team meetings
  • Ability to work independently while maintaining strong communication and responsiveness in a remote first environment

Job Duties and Responsibilities:

Team Leadership and Clinical Oversight (50%)

  • Lead and support a remote team of 10 to 15 Diabetes Educators and Dietitians, ensuring delivery of high quality, patient centered care
  • Conduct regular one on one meetings, coaching sessions, and performance reviews to support professional growth and accountability
  • Monitor team performance, productivity, and documentation quality to ensure adherence to clinical best practices, company protocols, and quality standards
  • Foster a collaborative and supportive team culture that encourages communication, accountability, and continuous improvement
  • Provide guidance and support for clinical, operational, and interpersonal challenges, escalating issues appropriately when needed
  • Partner with leadership and HR to support onboarding, training, mentorship, and ongoing team development
  • Ensure the clinical team remains current on diabetes care guidelines, Medical Nutrition Therapy standards, telehealth workflows, and internal quality initiatives
  • Support quality improvement efforts and contribute to operational process enhancements across the organization

Direct Patient Care (20%)

  • Provide virtual patient care through diabetes education consultations, nutrition counseling, and follow up visits
  • Develop individualized care plans focused on diabetes self management, nutrition, lifestyle modification, and sustainable behavior change
  • Educate participants on blood glucose management, medications, meal planning, exercise, and diabetes related technologies
  • Collaborate with patients to establish realistic goals and monitor progress over time
  • Interpret glucose and CGM data to support patient education and improve outcomes
  • Maintain accurate and timely documentation within the electronic health record (EHR)
  • Deliver a compassionate, professional, and culturally responsive patient experience

Collaboration & Project Management (30%)

  • Partner with leadership to implement strategic initiatives aimed at improving patient outcomes, operational efficiency, and team performance
  • Contribute to the development and refinement of clinical workflows, training materials, protocols, and patient education resources
  • Participate in leadership and operational meetings focused on quality initiatives, patient satisfaction, and process improvement
  • Analyze key performance indicators (KPIs), quality metrics, and team trends, providing recommendations to optimize care delivery and patient experience
  • Support implementation of new programs, technologies, and operational processes across the clinical team
  • Collaborate cross functionally with clinical and operational stakeholders to support continued organizational growth

Education and Credentials

  • Registered Dietitian (RD/RDN) with active licensure required.
  • Certified Diabetes Care and Education Specialist (CDCES) credential required.
  • Active professional licensure where applicable based on state requirements
  • Current certifications and licenses must be maintained throughout employment

Experience

  • Minimum of 3 to 5 years of experience in diabetes education, nutrition counseling, or related clinical care.
  • Minimum of 2 years of leadership, supervisory, or team management experience preferred.
  • Experience delivering patient care in a telehealth or virtual care environment strongly preferred.
  • Experience leading remote clinical teams preferred.

Clinical and Leadership Skills:

  • Strong knowledge of diabetes management, Medical Nutrition Therapy, and evidence based patient education strategies
  • Experience educating patients on diabetes medications, insulin therapy, and diabetes related technologies
  • Strong leadership, coaching, and mentorship abilities
  • Excellent communication, collaboration, organizational, and conflict resolution skills
  • Ability to build rapport quickly and maintain strong relationships with both patients and team members
  • Ability to manage multiple priorities in a fast paced remote work environment
  • Strong clinical judgment, professionalism, and accountability
  • Knowledge of HIPAA and data privacy best practices
  • Bilingual in English and Spanish or other languages is a plus

Additional Requirements:

  • Ability to provide telehealth services in accordance with state regulations
  • Willingness to obtain additional state licenses if needed to support patient demand
  • Ability to obtain payer credentialing if required
  • Must maintain active certifications and licensure
  • Must be authorized to work in the United States
