The Case Manager plays a crucial role in supporting youth and families by linking them to community resources, engaging youth in prosocial activities, supporting life skills development, and empowering families to engage with their local community.


ESSENTIAL FUNCTIONS:

  • Case Management:
    • Engage with youth and families non-judgmentally.
    • Conduct comprehensive intake assessments with families and adolescents, including prosocial inventories and community needs evaluations.
    • Conduct Case Management Session Rating weekly
    • Provide tailored community resources based on the specific needs identified through assessments.
    • Conduct follow-ups to ensure families are successfully connected to community resources.
    • Engage youth in prosocial activities in their local community to meet their individual needs.
    • Schedule and conduct in-home visits to deliver services.
    • Be flexible with scheduling to include conducting home visits in the evening to accommodate families’ schedules.
    • Advocate for youth across various settings, including schools and juvenile courts, as needed.
    • Assist families in exploring educational options and facilitate school enrollment for youth.
    • Maintain detailed and accurate documentation to ensure compliance with VisionQuest, state, and federal regulations. 
    • Complete documentation within 48 hours of service.
    • Develop and lead educational groups to include life skills and job readiness skills in person and via telehealth.
    • Teach essential life skills to adolescents individually  as needed.
    • Participate in weekly staffing meetings and other supervisory-directed meetings.
    • Participate in team after hours support rotation, providing immediate response to urgent needs and maintaining continuity of care outside scheduled sessions.
  • Communication and Coordination:
    • Maintain consistent, professional communication with assigned therapists and referral agents and community partners.
    • Meet weekly with team to staff cases.
    • Meet weekly with individual therapists to collaborate on treatment planning.
    • Work together with therapist to identify goals to meet the client’s needs.
    • Focus on case management goals and maintain boundaries within the case management scope of practice (community resources, prosocial activities, life skills, setting concrete goals related to education, career, prosocial activities,  etc.)
    • Attend at least three (3) community events per year on a Saturday or Sunday to represent the program and engage with  the community.
  • Logistics and Travel Arrangements:
    • Transport youth to necessary appointments and court appearances.
    • Educate youth on how to navigate public transportation systems.
  • Compliance:
    • Accurately document all case management activities using the appropriate systems.
  • Other tasks and responsibilities as assigned by supervisors.


SKILLS & ABILITIES: 

  • Excellent interpersonal communication skills with ability to listen actively. 
  • Commitment to professional development and proactive research.

  • Cultural humility when working with a diverse population
  • Ability to work independently and as part of a team. 
  • Strong computer skills (Microsoft Office, Google Workspace, Google Drive) with the ability to learn new software.


EDUCATION & EXPERIENCE: 

  • Bachelor’s degree in the behavioral sciences, human services or social services fields required
  • Bachelor’s degree in Social Work preferred. 
  • Child welfare and/or case management experience is strongly encouraged.
  • A minimum of one year of related work experience, preferred.



QUALIFICATIONS: 

  • Must be 21 years of age or older.
  • Bilingual in English and Spanish is preferred.
  • Flexible schedule (evening, weekends, holidays) to facilitate program services.
  • Must be able to pass a drug test.
  • Cleared background checks from appropriate entities, including Child Abuse and Neglect and FBI criminal background.
  • Must have a valid driver’s license and be eligible to drive to facilitate program services as required.



WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

  • May lift and carry up to 10 pounds frequently
  • Performs tasks that may involve standing or walking 50% of the day; sitting 50%of the day.
  • May bend, stoop, reach and grasp as necessary.
