About the role
What you'll do
PRIMARY RESPONSIBILITIES:
Evaluates and treats center patients in accordance with standards of care.
Assists in the coordination of patient services, including but not limited to specialty referrals when appropriate, cardiology subspecialists and vascular surgeons, etc.
Acts as an active participant and key source of medical expertise with the care team.
Partners with the primary care physicians and nurse practitioners in a tone of cooperation in practice by displaying a professional and approachable demeanor
Completes all medical record documentation daily working with a quality- based coder to optimize comprehensive documentation.
Follows policy and protocol defined by Clinical Leadership
Meets with primary care physicians (including nurse practitioners) and market medical director and about quality of care, review of outcome data, and medical records issues.
Clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care
Support our longitudinal care model for heart failure with focus on disease management and prevention.
Support readmission reduction initiatives for heart failure and CAD.
Create a practical framework for upstream stroke prevention.
Help coordinate care for CKD stage 5 patients needing AV graft placement prior to initiating hemodialysis.
Interpret and document in detail test results to determine how effectively the heart and or cardiovascular system is functioning i.e., Echocardiograms, stress test, etc.
Coordinate with PCP
Coordinate patient care with cardiology subspecialist i.e., interventional, EP, Thoracic, etc.
Qualifications
REQUIRED QUALIFICATIONS:
Graduate of accredited MD or DO program of accredited university.
Current and unrestricted medical license or willing to obtain a medical license in state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.

Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients.
