Case Manager Coordinators– CalAIM ECM & Community Supports

The Case Manager for CalAIM Enhanced Care Management (ECM) and Community Supports provides comprehensive, person-centered care coordination to Medi-Cal members with complex medical, behavioral health, and social needs. This role works collaboratively with healthcare providers, community-based organizations, and social service agencies to address health-related social needs and improve health outcomes in alignment with CalAIM program requirements.

The Case Manager serves as a primary point of contact for members, ensuring timely access to services, continuity of care, and advocacy across medical, behavioral health, and social systems.

Key Responsibilities

Care Coordination & Case Management

  • Conduct comprehensive assessments addressing medical, behavioral health, social determinants of health, and functional needs.
  • Develop, implement, and monitor individualized care plans in collaboration with members and interdisciplinary care teams.
  • Coordinate ECM services including referrals, follow-ups, and ongoing engagement with members.
  • Support transitions of care (e.g., hospital discharge, skilled nursing, housing placement).

Community Supports Navigation

  • Connect members to CalAIM Community Supports such as housing navigation, housing deposits, medically tailored meals, transportation, respite care, and recuperative care.
  • Collaborate with community-based organizations and service providers to ensure timely service delivery.
  • Track service utilization and outcomes per CalAIM guidelines.

Member Engagement & Advocacy

  • Build trusting relationships using trauma-informed, culturally responsive, and person-centered approaches.
  • Advocate for members to reduce barriers to care and improve access to needed services.
  • Provide education on health conditions, benefits, and available resources.

Documentation & Compliance

  • Maintain accurate, timely documentation in electronic health records and care management systems.
  • Ensure compliance with CalAIM, Medi-Cal, and organizational policies and reporting requirements.
  • Participate in audits, quality improvement initiatives, and team meetings.

Qualifications

Required:

  • Bachelor’s degree in Social Work, Human Services, Public Health, Nursing, or related field (or equivalent experience)
  • 1–3 years of case management or care coordination experience
  • Knowledge of Medi-Cal, CalAIM, or similar programs
  • Experience working with high-risk or vulnerable populations
  • Strong communication and organizational skills

Preferred:

  • Licensed or certified professional (LCSW, RN, LVN, CHW, etc.)
  • Bilingual/bicultural a plus
  • Familiarity with community resources and managed care
