About the role


Lead successful development, implementation, management, and process improvement for a TH+ Care Hub. Promote and leverage technology to support Care Hub skin & wound care coordination between patients, care circles, staff, partners, and others, to achieve seamless collaboration, efficient workflows, and improved patient outcomes. Empower a cross-functional team approach to achieve growth and success of virtual and on-site care coordination, revolutionizing the way skin and wound care is delivered.


Key Responsibilities:

Strategic Clinical Coordination

  • Implement and manage Care Hub clinical coordination strategies, encompassing:
    • Comprehensive staffing and onboarding processes
    • Training and team competency development
    • Supply management and budget oversight
    • Care planning approvals
    • Day-to-day operational functionality

Performance and Outcome Management

  • Develop and track clinical outcome metrics to:
    • Evaluate program effectiveness
    • Identify improvement opportunities
    • Recognize and celebrate team successes

Collaborative and Stakeholder Engagement

  • Collaborate extensively with:
    • Care Hub team
    • Social workers
    • Community partners
    • Healthcare organizations
  • Support clinical staff in addressing:
    • Social determinants of health
    • Health disparities
    • Language barriers
    • Specialized clinical needs

Continuous Improvement and Innovation

  • Identify and implement opportunities for Care Hub enhancement
  • Ensure alignment with:
    • Evidence-based practices
    • Regulatory requirements
  • Evaluate and evolve care coordination workflows
  • Develop cross-functional team improvement strategies

Staff Support and Development

  • Create comprehensive staff support programs focusing on:
    • Work-life balance
    • Staff wellness

Performance Monitoring and Quality Assurance

  • Create and maintain performance metrics and KPIs
  • Compare program performance across Care Hubs
  • Identify and address operational challenges
  • Implement process improvements

Relationship and Communication Management

  • Build and maintain relationships with:
    • Healthcare providers
    • Specialty groups
    • Community organizations
    • National and regional healthcare associations
  • Represent organization on targeted boards and committees
  • Foster a culture of growth, compassion, and accountability

Clinical Oversight and Care Coordination

  • Provide critical oversight for:
    • Care planning
    • Risk factor adjustments
    • Episode and encounter modifications
  • Ensure seamless, patient-centered care coordination
  • Review and approve formulary needs
  • Manage high-risk patient medication reviews
  • Maintain professional communication during provider escalations

Qualifications:

  • Nurse Practitioner license in Texas
  • Wound Care Experience required
  • CWOCN or CWS preferred
  • Extensive experience in clinical care coordination
  • Strong understanding of healthcare regulations and best practices
  • Excellent communication and leadership skills
  • Proven ability to develop and implement strategic initiatives
