We are seeking a compassionate, skilled Clinical Social Worker to join our wound care team. In this role, you will work to deliver high-quality, patient-centered psychosocial care within the THP Care Hub by addressing social determinants of health (SDOH), behavioral health needs, and care integration. Provide clinical expertise in mental health, behavioral change, and community resource navigation to optimize patient outcomes. Empower patients and caregivers through education, emotional support, and coordinated care planning. Serve as the designated mental health and social work clinician within the pod, ensuring an interdisciplinary, whole-person approach to care.
Key Responsibilities
Patient-Centered Psychosocial Assessments and Interventions
- Conduct comprehensive patient-centered assessments to identify behavioral health risks, social determinants of health barriers, and care needs
- Implement evidence-based behavioral interventions to support mental health, treatment adherence, and lifestyle changes
Behavioral Health and Emotional Support
- Provide brief, solution-focused behavioral health interventions for patients experiencing wound-related distress, anxiety, depression, or non-adherence barriers
- Facilitate caregiver and family support groups to address stress, burnout, and patient care challenges
- Act as a psychosocial expert in interdisciplinary team meetings, integrating behavioral health perspectives into care plan discussions
- Coordinate with external mental health providers when higher-level care is needed
Caregiver Support and Patient Education
- Design and lead evidence-based psychoeducation sessions on topics such as chronic disease self-management, stress reduction, wound care adherence, and mental health awareness
- Implement caregiver support groups to enhance caregiver resilience and patient adherence
- Partner with leadership to develop patient education materials focused on behavioral health and wound care compliance strategies
Community Resource Integration and Social Care Navigation
- Identify, coordinate, and track community-based services that address housing, transportation, food insecurity, and financial barriers
- Manage community contracts and partnerships to expand referral pathways for patient support
- Serve as the care pod leader on SDOH Risk Assessment screening, community health integration, and resource linkage to enhance care coordination
Interdisciplinary Collaboration and Leadership
- Utilize AI-generated SDOH analytics and predictive modeling to identify high-risk patients and tailor interventions
- Monitor psychosocial impact metrics (e.g., adherence rates, SDOH Risk Assessments, engagement) to refine interventions
- Collaborate with the Care Hub team to analyze patient progress, improve workflows, and contribute to process improvement initiatives
Qualifications:
- Master’s level licensed social worker (LMSW, LCSW) with experience in healthcare, behavioral health or community-based settings.
