We're seeking a highly skilled and compassionate Wound Care Specialist to join our innovative team.

In this role, you'll:

• Provide expert skin and wound care through comprehensive assessments
• Lead interprofessional care planning teams, ensuring top-quality patient care
• Empower patients to take an active role in their healing journey
• Coach and support Care Navigators in delivering excellent patient care
• Utilize cutting-edge AI and wound care technologies to improve patient outcomes

Key Responsibilities:

• Conduct virtual assessments and support for patients and Care Navigators
• Perform comprehensive wound assessments using advanced technologies
• Lead Care Planning 360 sessions and regular care plan reviews
• Provide patient education and support, promoting active participation in care
• Analyze care metrics and contribute to continuous improvement initiatives
• Stay updated on the latest wound care research and best practices

Qualifications:

• Valid RN Licensed in Texas (or compact license) required
• Home Health Experience required
• Experience in telehealth or remote patient monitoring is a plus
• CWS or WOCN with at least 3 years of experience required
• Strong clinical expertise in evidence-based wound care practices
• Excellent communication and leadership skills
• Proficiency in using healthcare technologies
• Commitment to patient-centered care and continuous learning
