Job Description:

The Integrated Community Health Worker (ICHW) is an essential role that supports patients by connecting them to appropriate Behavior Health and Primary care services and resources, provides individualized care coordination for patients with unmet, complex, or ongoing medical needs to ensure communication between all providers regardless of their discipline, follows up with patients to help them overcome barriers to successfully implement their action plan or treatment recommendations as needed,  and monitors the accuracy of all aspects of the patient’s chart as stated on the Integrated Service Plan (ISP).  The ICHW will become trained in and lead interventions on Chronic Disease Self-Management, will be responsible for connecting with patients and coordinating with staff to maintain documentation compliance and service delivery, and must have strong organizational and administrative skills to support monitoring expiring and inaccurate documentation.

Job qualifications
  • Must be 18 years old; and 
  • High School diploma/GED and 6 months of BH work experience (BHT I) and 1 year of PCP experience required
OR
  • Behavioral Health Technician II (BHT II)
  • 18 Years of age or older; and
  • 1 year of PCP experience; and
  • Bachelor’s or master’s degree in behavioral health (B.H.) related field; or
  • Clinical Intern in bachelor’s or master’s degree behavioral health program in good standing; or
  • Bachelor’s or master’s degree in non-BH related field AND 1 year of BH work experience; or
  • Associate’s degree AND 2 years of BH work experience; or
  • High School diploma and 4 years of experience in a BH setting (1 year must be direct patient contact). If the applicant has less than four years and/or at least one year of direct care experience, Director approval will be required during hiring and credentialing process.
  • Must have the skills and knowledge necessary to provide the behavioral health services that the agency is authorized to provide and to meet the unique needs of consumer populations served by the agency, including children and meet requirements for a Behavioral Health Technician I or II.
  • The ability to manage multiple changing priorities in an effective and organized fashion.
  • Ability to provide culturally sensitive services
  • Strong interpersonal, verbal, written, and communications skills.
  • Knowledge of MS Windows 365 environment and/or Electronic Health Records Systems. A
  • Ability to provide culturally sensitive services.
  • Must have or be able to obtain a valid Fingerprint Clearance Card.
  • Must possess reliable transportation, valid driver's license, and proof of auto insurance.
  • Basic Life Support trained or the ability to be trained within 10 business days from hire date.
  • TB Screening Record and PPD/QuantiFERON/Chest x-ray upon hire (as applicable).
  • Vaccinations/Titer’s Test/Declination Form: Flu, Hep B, MMR, Tdap, Varicella.
  • Covid 19 Vaccination or Qualified Exemption (Religious or Medical).
