Job Description:


This individual provides case management, care coordination and psychiatric services for Adults with a Serious Mental
Illness. The Case manager-CC will provide ongoing assessment, treatment planning, service coordination with physical and
mental health systems, and other activities as identified by the clinical Team. The Case manager-CC will facilitate the court ordered treatment process, including obtaining specific information from clinical teams/staff and/or case management system; will assist with letter of intent to treat, subpoenas, witness notifications, and research support; will provide quality control of all submitted documents for court filing from the clinics and coordinate revisions as applicable.

Job qualifications:

  • 18 years of age or older; and
  • Bachelor’s or master’s degree in Behavioral Health (BH) related field; or
  • Clinical Intern in bachelor’s or master’s degree BH program in good standing; or
  • Bachelor’s or master’s degree in non-BH related field AND 1 year of direct patient contact in a healthcare setting; or
  • Associates degree AND 2 years of direct patient contact in a healthcare setting; or
  • High School diploma and 4 years of experience in a healthcare setting (1 of the 4 years must be direct patient contact) **If applicant has less than four years and/or at least one year of direct care experience, Director approval will be required during hiring and credentialing process.
  • Strong verbal, written, and communications skills
  • Knowledge of Windows environment and/or Electronic Health Records Systems.
  • Ability to provide culturally sensitive services
  • Must have or be able to obtain a valid Fingerprint Clearance Card.
  • Must possess reliable transportation, valid driver's license, and proof of auto insurance.
  • Must maintain current BLS certification
  • Covid 19 Vaccination or Qualified Exemption (Religious or Medical).
  • TB Screening Record and PPD/QuantiFERON/Chest x-ray as applicable upon hire
  • Vaccinations/Titer’s Test/Declination Form: Flu, Hep B, MMR, Tdap, Varicella
