Tuesday Health launched in 2023 to deliver compassionate, supportive care for patients and caregivers navigating serious illness. We believe that each patient's journey is unique. As such, we deliver member-directed, whole-person care to seriously ill patients in our program, using leading edge supportive care models, including appropriate transitions to hospice when the time is right. This results in dramatically improved quality of life for members and their caregivers, and a meaningful reduction in unproductive medical spend. Our overall mission is to transform serious illness and end of life care; our team believes deeply in this mission and puts our members first in all that we do.
The Role
This full time NP role is a 100% clinical position that provides oversight to our nursing team in addition to providing direct clinical care of patients through telemedicine and in-person visits. This position will travel within the community to support our clinical team and patients.
Key responsibilities may include:
· Responsible for ensuring the highest quality delivery of supportive/palliative care to people with serious illness and their families.
· Provide clinical supervision for clinical care team members in the assigned market/region as indicated, particularly medium and high-risk patients.
· Establish collaborative relationships through outreach with primary care physicians, facility providers, and other community providers and specialists, to provide the best care possible that results in a cohesive coordinated approach to optimal care for each patient
· Help lead interdisciplinary care team meetings with a focus on reviewing complex cases, patients with high-utilization patterns or those nearing hospice eligibility, and relevant clinical education in the context of case discussions.
· Perform palliative care consultations in person and using telemedicine, particularly for high-risk or high-utilization members, when indicated.
· In partnership with clinical leadership, present on educational topics such as serious illness communication, advanced illness management and symptom management, among others
· Provide administrative and clinical guidance and support to the care team to ensure quality outcomes for patients and families and ensure clinicians are operating at the top of their licensure
· Periodically review and approve care team clinical documentation and orders, and provide coaching feedback for areas of clinical competency and accuracy
· Provides medical oversight/consultation for licensed practical nurses and registered nurses.
· Prescribes medications when needed to avoid a utilization event in accordance with Tuesday Health prescribing policy.
· Provide rotational on-call coverage after hours with other regional and/or local providers for care escalation as required to prevent unnecessary or avoidable hospitalizations and ED visits
